How to get Support for You or Someone Else

Are you in need of Help? Or do you know a Youth who may need our support? Check out the information below on how to refer a Youth to our Services.

We want to make sure that our Programming and Supportive Services are accessible to the Youth who may need them. If ever you are unsure about the information on Programming, Admission Requirements, or How to Refer do not hesitate to reach out to us: admissions@southwestyes.com, (306) 773-0567

Inpatient and Outpatient

Are you are a Youth (12-18 years old) and residing in Saskatchewan?

  • Review our Admission Requirements. Reach out to us if you have any questions or if you aren’t sure you qualify for our services.
  • Read our ‘Participant Manual’ so that you understand our structure, how to prepare, your rights/responsibilities and what to expect. Reach out if you have any questions and we would be happy to answer them for you.
  • Talk to your Parent(s)/Guardian(s) and share your interest in attending Treatment. If you are 12-15 years old, you will need their Consent. Reach out to us if you need help with this conversation or your options if your parent(s)/guardian(s) don’t agree with Treatment.

If you have read the Partcipant Manual and your Parent(s)/Guardian(s) are on-board, send us an e-mail (admissions@southwestyes.com) or call and leave us a message (306-773-0567) letting us know you are interested in treatment and include the following information:

  • Your Full Name, Birth Date and Health Number
  • Your Full Address (where you are living)
  • Your Phone Number

We will Call you Back to go through an ‘Intake Process’ (What does that mean? We will ask you some questions to understand your situation and your need so that we can explore your options and see how we may support you).

Are you are a Parent/Guardian of a Youth (12-18 years old)?

  • Review our Admission Requirements. Reach out to us if you have any questions or if you aren’t sure your Youth qualifies for our services.
  • Talk to your Youth and explore their interest in attending Treatment. You can review the ‘Participant Manual’ with them so that you can discuss it together. Reach out to us if you need help with this conversation. In order to be accepted into Treatment the Youth must be willing and voluntary.

If you have read the Participant Manual and your Youth is on-board, send us an e-mail (admissions@southwestyes.com) or call and leave us a message (306-773-0567) with the following information:

  • Your Full Name and your relationship to the Youth (mom/dad, legal guardian)
  • The Youth’s Full Name, Birth Date and Health Number
  • Your Phone Number

We will Call you Back to go through an ‘Intake Process’ (What does that mean? We will ask you some questions to understand the Youth’s situation and your need so that we can explore theoptions and see how we may support you and the Youth).

Are you an Saskatchewan Health Authority Worker (Doctor, Nurse, Psychiatrist, Mental Health and Addictions Worker, etc.)

  • Review our Admission Requirements. Reach out to us if you have any questions or if you aren’t sure the Youth qualifies for our services.
  • Talk to the Youth and explore their interest in attending Treatment. You can review the ‘Participant Manual’ with them so that you can discuss it together. Reach out if you have any questions. In order to be accepted into Treatment the Youth must be willing and voluntary.

If you have read the Participant Manual and your Youth is on-board:

  • MHAIS Users: Follow steps in applicable SHA Work Standard: 1.)’Referral to SWYES (Dorie’s House) Residential Treatment’, 2.)’Referral to SWYES (Dorie’s House) Outpatient Treatment’.
  • Non-MHAIS Users: Follow step in applicable SHA Work Standard: ‘Referral to SWYES (Dorie’s House) from non-MHAIS Users’.

Do you know a Youth who may be in-need of our Services but you are not a Parent or Legal Guardian (You may be a teacher, friend, family member, pastor, or someone else who cares about the Youth)

  • Review our Admission Requirements. Reach out to us if you have any questions or if you aren’t sure the Youth qualifies for our services.
  • Read our ‘Participant Manual’ so that you understand our structure, and whether it might be a good fit for the Youth you are thinking about. Reach out if you have any questions and we would be happy to answer them for you.
  • Talk to the Youth and/or the Parent(s)/Guardian(s) of the Youth and share your thoughts on the Youth’s need for our services. Reach out to us if you need help with this conversation.

Refer the Youth and/or the Parent(s)/Guaridan(s) of the Youth to the Referral information listed above.
